The following guidance resources are available for supervisors:

Reporting forms

Use the appropriate form to report on a doctor who is working under supervision.

Run reports

• Beginning of run objectives and mid-run assessment RP1 (pdf)
• End of run assessment for interns and NZREX graduates RP2 (pdf)

3 monthly report

• Supervisor’s report: provisional general, special purpose, temporary registration RP3 (pdf)
• Supervisor’s report: provisional vocational RP4 (pdf)

Changing scope

Supervisors/employers will also provide input to the following applications:

• During Supervision - if a doctor is changing their supervisor or employer.
• At the end of this supervised period - if a doctor wishes to progress to general or vocational scope.
