Changing your employment

It is important that you let us know when you change your job, so that we may always have the most up-to-date employment and contact information for you. In some cases you will also need us to approve a change of employment before you take up a new role. The process you will need to follow depends on the type of registration that you hold.

Vocational and / or general registration

Provisional general, provisional vocational or special purpose registration

If you hold this type of registration, then you must contact us before you change employment, because you will need to apply to change your scope of practice on your practising certificate (PC) and be issued with a new PC. You can’t change jobs until your PC has been changed to include the details of the new employment, including where you will be working and who your new supervisor will be (if applicable). Select from the list below to find out how to apply to change your scope of practice.

Provisional general:

If you have a New Zealand or Australian primary medical degree, or if you passed the NZREX exam:

  • A completed COS1 form, signed by:
    • yourself
    • your proposed Intern Supervisor
    • your CMO/Practice Principal; and
    • your employer.
  • A copy of your letter of employment
  • Up to date supervision reports

If your primary medical qualification and internship were completed in the UK or Ireland:

  • A completed COS1 form, signed by:
    • yourself
    • your proposed supervisor  - must be vocationally registered in the same or similar branch of medicine
    • your CMO/Practice Principal; and
    • your employer.
  • A copy of your letter of employment
  • A supervision plan (if working in primary care)
  • Up to date supervision reports

If you have previously worked in a comparable health system:

  • A completed COS1 form, signed by:
    • yourself
    • your proposed supervisor - must be vocationally registered in the same or similar branch of medicine
    • your CMO/Practice Principal; and
    • your employer.
  • A job offer
  • Up to date supervision reports
  • A new supervision and induction plan

Provisional vocational:

Each individual case is different. Please contact the Vocational Registration Team for more information.

Special purpose (locum tenens) registration:

If you are wishing to change employment under special purpose (locum tenens) registration, you will need to submit the following to the Vocational Registration Team:

  • A completed COS7 form, signed by
    • yourself
    • your proposed supervisor - must be vocationally registered in the same or similar branch of medicine
    • your CMO/Practice Principal; and
    • your employer.
  • A copy of your letter of employment.
  • Up to date supervision reports
  • A new supervision plan

Please note that under special purpose (locum tenens) registration, you may only work a maximum of 12 months within an 18 month period. You will need to ensure that your new employment does not exceed this timeframe, after counting the time you have already worked with your initial employer.

Special purpose (Postgraduate Training) registration:
