If you want to make a notification about the treatment you have received from a doctor your first option might be discussing the matter directly with your doctor or the relevant practice or hospital.

The Health & Disability Commissioner provides these steps help you to be clear about your concerns and how you want to resolve them. Go through these steps first, then contact the person or organisation who provided the service.

The Nationwide Health and Disability Advocacy Service is a free service that operates independently from all health and disability service providers, government agencies and Health and Disability Commissioner. If you want to know more about your rights when using health or disability services, get questions answered, or make a notification, they can help

This is free of charge and they will put you in touch with a local patient advocate who will help you understand your rights and advise you on the best way to make a notification.
